Enter a subject, or select from the pre-defined list of subjects.

This list is defined by the Loan Log Default Interest Rate Review Subjects Information List.

Enter any applicable notes about the Default Interest Rate change.

Optionally enter the date this Log should be actioned.

Optionally enter a time.

Select a Log Severity from the list.
