Date

Enter the date the log is to be dated. This will default with today's date.

Enter a subject or select from the pre-defined list of subjects.

This list is defined by the Account Log Stmt Subjects Information List.

Enter any applicable notes about the Statement change.

Optionally enter the date this Log should be actioned.

Optionally enter a time.

Select a Log Severity from the list.
